I do agree with you and the cop out is generally "oh well, there are exceptions" etc. Most times things were done correctly and probably 75% of the exceptions are after the fact changes. Agreed. 

All that being said, I do know of at least two significant exceptions personally. One is the lack of a rim blow horn on the loaded Donohue car and the other is that my own car came originally with no engine call out on the driver's side. I got it as an original paint car with original fenders and there is no holes, no emblem, nothing. It did happen, but not as often as people would like to think, especially when they are justifying a change for whatever reason!
