There are many options out there. For my '69 AMX I'm running Moser 1 piece axles with the Wilwood Kit (sourced from Control Freak (freakride.com). Brackets are included with that kit. On my '70 Javelin (street car turned Trans-AM race car), they are custom brackets that the fabricator back around 1975 made out of aluminum plate to fit C3 Corvette Calipers. You'll find some kits online from other aftermarket suppliers to the AMC family, scarebird.com is a popular one.

PS-you might move this topic to the suspension and brakes category to catch some more replies.

On my Pro Rally SX/4 401 V8 I built custom brackets welded on the axel.
I used eagle front calipers and rotors. The hub had to be turned down to fit the rotors. I used two inside pads from the eagle which are smaller than the outer pads since the rotors front and rear were the same size. I also added a proportioning valve for the rear brakes.
A lot of work but cheap and they worked well. Also used a disc-disc master.

I ran rear discs from TSM on my Jeep AMC20.  Loved them.  They will sell you just the brackets if you call.
